Registration deadline:
Registration forms must be submitted to Houston Tennis Academy one week in advance to secure spot in the academy.
Method of Payments:
Houston Tennis Academy accepts personal checks, money orders, and credit card payments.
Payments must be received no later than first day of attendance.

Medical Release:
No application will be accepted without a completed Liability/Medical Release Form. Please download and complete the form, and submit to one of our Program Directors.
